Consent form for Online Sessions with Jeanette Amlie

SomaPsycheSoul Sessions utilize Somatic Processing, Creative Inquiry and Spiritual/Intuitive Support to assist you in discovering your natural capacity for healing and growth. It is a relatively simple process, but that does not necessarily mean that it is easy work. It does require a desire to discover and work with the causal factors underneath your symptoms and a commitment to self-care in between sessions. Because I often offer this process over the internet, it requires the participants to be willing and able to resource themselves and/or find additional local support if needed.

I am not a licensed counselor or trauma therapist, although I utilize skills and techniques from all of my training and experience. I do not diagnose or treat any condition or illness and this work is not intended to replace other forms of medical or psychological treatment. Please keep your regular healthcare practitioners advised of this work and seek their counsel when necessary.

Online sessions are held online via video conference and require high speed internet, web camera and stereo earbuds or headphones.

The fee for a one hour session is $60 USD. A discounted rate of $150 is available for people doing 3 sessions within a 30 day period. Sessions may be rescheduled with 24 hours notice, but payments are non-refundable. All scheduled sessions must be confirmed with payment 24 hours in advance.
